Assessment

Before signing a contract with an agency of health care, the assessment will be to identify needs of the person requiring care and the prescription of a medical home health is not fully expose all your needs. The evaluation provides the basis for a care plan. A care plan is a document that explains the client's needs, and how to address them. A care plan should be reassessed by an agency on a monthly basis and when necessary. A care plan:

* Increases consistency of care (this is extremely important when multiple caregivers must interact with each other)
* Set priorities and goals for the care of a person
* Acts as a benchmark to help measure the progress of an individual

Contract

Home care agencies health will have you sign a contract. These contracts can be changed and changed, so before signing it, think about whether there are certain verbal promises that should be written into the contract. For example, if an agency promises you that a replacement doctor is always within a specified period, if the primary caregiver is sick or be absent for a day, you may want to have the agency add that detail the contract.
